Stopgate Lane Medical Centre is involved in the care of patients in a specified local area, and is available to anybody residing in this area. For new patients wanting to register, please see the map on the back cover.

You will be asked to complete a health questionnaire and provide some form of identification. You will also need to provide proof of your address. Everyone over 16 years of age will be given an appointment with a nurse. She will ask about your past medical history and current medication to assist in assessing your health needs. A brief health check will be performed so please bring a specimen of urine with you. In addition you will be asked to provide details of childhood immunisations of children under six years of age (this will ensure that your child is included in the computerised record of the local Child Health Department).

The government has made some provision for patients who wish to utilise our service even if they are not in the specified area. This may be of benefit to patients who may work in the area or other reasons. This is a restricted service which, for example, won't include home visits. This is a new service in its infancy and we would welcome any view you may have especially if you think you may be in this category. Your views would help us to ascertain the level and type of demand and support us in evaluating this new service.
